I hereby authorize the staff of Rising Star Baseball Camp to act for my son/daughter in their best judgment in any emergency situation. I also authorize my son/daughter's attendance at the Rising Star Baseball Camp. I certify that he/she is in good health and is able to participate in all camp activities. In case of emergency, I grant permission for my child to be given treatment as prescribed by a physician or hospital. I agree to hold Rising Star Baseball Camp LLC, Coach Darren Gurney, and his camp staff harmless from any liability, loss, or personal expense arising on the way to, during, or on the way home from the camp. I understand that there are no cancellations or transfers within thirty days of a given camp week.  I also give permission for the use of any photos and video taken at the camp for publicity purposes, NCAA scouting, purchase by family members, or internet camp web-site exposure, including, but not limited to, DVD & instructional book purposes.
